What forms will I need to fill out upon my visit?

You can find these forms on our web page. Just click on the link and print the form on your printer. To expedite your visit, please bring your completed forms with you to our office.

 
What will I need to bring on my visit if it is a Self Pay, Worker's Compensation or Personal Injury/Liability claim?

You will need the patient's social security number as well as payment or proper payment arrangements at time of service.  For more details, please refer to our self pay, worker's compensation, or liability sections in our financial policy.

 
How do I make an appointment?

Please call our offices to make an appointment. Appointments will not be accepted at this time via this web site. Our Springfield Office number is (877) 862-0624 or (217) 862-0624. Please view our locations for the phone numbers of our other offices. If you are interested in making appointments via the internet, please let us know by clicking here. In the future, we hope to accept appointments via the internet. For now, we would appreciate your feedback.

 
Where are you located?

Our Springfield Office is located at the corner of Old Jacksonville Road and Koke Mill Road which is a quarter of a mile west of Veterans Parkway (Route 4) on Old Jacksonville Road. The building sits in the south east corner of that intersection. Please view our locations section for detailed maps.

 
How do I arrange payment?

As a courtesy, we do submit insurance claims for our patients. It is important that we have your correct insurance information.  We also accept cash, checks, debit cards, and VISA or Master Card.  We can accept credit card payments by phone.

 

New Patients

Our patient accounts staff will be happy to discuss your account and arrange a payment plan with you upon your arrival at our offices.
